1) A student may enter a higher level division than their grade stipulates in the Competition Rules, but this requires the permission of their parent or teacher, and requires the parent's signature.

2) The Division and Category items in the above form can be found on the Competition Rules sheet.

3) The Medium category above should also be taken from the Competition Rules list. If the stated medium differs from those itemized in the Rules, the piece will be judged against the most similar medium in the Rules, at the discretion of the judges, unless considered too different from the choices listed in the Rules.

4) A Title is not required for entry or for judging, and will not affect the outcome of the judging. It is simply for presentation afterward and is up to the student who enters the piece.

5) A brief description of the pieces should be listed just in case there is some confusion as to which entry belongs with which piece.

6) The person who receives your entry will assign an Entry # for judging purposes.
